Medication Notice:

• Please consume and bring along any medication such as those for high blood pressure and any other chronic diseases on the day of your appointment.

Urine Test

• To be done at least 5 days after your menses (ladies)
• Urine specimen bottle will be provided on day of test, please collect mid-stream urine, half the bottle

X-Ray

• Not recommended if pregnant
• Please inform our doctor if you had a chest x-ray in the preceding 6-months.

Vision Test/ Color Vision Test

• Please bring along your spectacles or contact lenses if you have any visual disorder such as short sightedness.

For LTA Exam:

• Please ensure that you are notified for a LTA examination.
• Kindly bring along your LTA examination form on the day of your examination.
• Please consume and bring along any medication such as  those for high blood pressure and any other chronic diseases on the day of your appointment.
• Please bring along your spectacles or contact lenses if you have any visual disorder such as short sightedness.
