Preparation for Health Screening
What should I bring for my health screening appointment?
Please remember to bring along all your necessary employment medical forms.
Please bring along your Passport and NRIC/FIN card (if applicable)
What should i take note of for my appointment?
Please postpone your appointment if you are unwell.
You are strongly encouraged to come at your registered time slot.
You will receive an SMS reminder if you have provided us a valid mobile number.
Test specific preparations:
Medication Notice:
Please consume and bring along any medication such as those for high blood pressure and any other chronic diseases on the day of your appointment.
Urine Test
To be done at least 5 days after your menses (ladies)
Urine specimen bottle will be provided on day of test, please collect mid-stream urine, half the bottle
X-Ray
Not recommended if pregnant
Please inform our doctor if you had a chest x-ray in the preceding 6-months.
Vision Test/ Color Vision Test
Please bring along your spectacles or contact lenses if you have any visual disorder such as short sightedness.
For LTA Exam:
Please ensure that you are notified for a LTA examination.
Kindly bring along your LTA examination form on the day of your examination.
Please consume and bring along any medication such as those for high blood pressure and any other chronic diseases on the day of your appointment.
Please bring along your spectacles or contact lenses if you have any visual disorder such as short sightedness.
We regret to inform you that should the above not be satisfied, we would not be able to process the examination for you.
